چکیده

Frontal plane humeral elevation effects on the pullout strength of suture anchors used in rotator cuff repair The rotator cuff offers the stabilizing forces necessary for fine arm movement. The most common injury to the rotator cuff is the partial or full thickness tear of the supraspinatus tendon, which attaches the humerus to the scapula on the superior side, due to an acute or chronic injury. These injuries account for more than 4.5 million physician visits per year. Depending on the degree of tear, arthroscopic surgery may be needed to reattach the torn tendon to the humeral head. The current preferred method of surgery involves the insertion of a suture anchor into the humeral head that ties the detached tendon using sutures back to bone. Usage of suture anchors promotes healing of the supraspinatus tendon onto the bone re-establishing movement of the arm. Although they provide a suitable healing interface, suture anchors are prone to various types of failure such as anchor eyelet breakage, suture failure, and anchor pullout. These failure modes necessitate intervention to reattach the tendon thus prolonging the healing period. Identifying and minimizing these failure modes will ensure optimum healing. There are two goals of this study. Firstly, the paper will validate suture anchor pullout test methodology in comparison with previous studies using polyurethane blocks to ensure robust methods and results. Secondly, this study will investigate the effects of frontal plane humeral elevation on the pullout force of suture anchors. v ACKNOWLEDGMENTS

The Effect of Humeral Elevation Angle on the Pullout Strength of Suture Anchors

INTRODUCTION: Rotator cuff injuries account for more than 4.5 million physician visits annually in the U.S. Injuries to the rotator cuff are typically treated arthroscopically by using implanted suture anchors for the repair. Common suture anchor designs include variations in thread design and the material used. The primary objective of this study was to understand and model the effects of fron...

In vivo microstructural analysis of the humeral greater tuberosity in patients with rotator cuff tears using multidetector row computed tomography

BACKGROUND In arthroscopic surgery, the suture anchor technique has become popular for rotator cuff repair. Preoperative evaluation of the bone microstructure is of utmost importance because, especially in elderly patients, osteoporotic changes may cause anchor pullout, which results in failure of rotator cuff repair. Many groups have reported humeral microstructural analysis; however, most stu...

Pullout strength of suture anchors used in rotator cuff repair.

BACKGROUND Surgical treatment of rotator cuff tears may be complicated by osteoporosis of the proximal part of the humerus. The purpose of this study was to determine whether pullout strength of suture anchors is affected by the location of the anchor placement and by bone mineral density. We hypothesized that higher bone mineral density is associated with higher pullout strength of suture anch...

Mechanical properties of all-suture anchors for rotator cuff repair

OBJECTIVES All-suture anchors are increasingly used in rotator cuff repair procedures. Potential benefits include decreased bone damage. However, there is limited published evidence for the relative strength of fixation for all-suture anchors compared with traditional anchors. MATERIALS AND METHODS A total of four commercially available all-suture anchors, the 'Y-Knot' (ConMed), Q-FIX (Smith ...

Biomechanical performance of rotator cuff repairs with humeral rotation: a new rotator cuff repair failure model.

BACKGROUND Traditional biomechanical evaluations of rotator cuff repair techniques employ cyclic loading of the supraspinatus tendon in an isolated medial direction. PURPOSE This study was conducted to evaluate 2 different rotator cuff repair techniques that are currently the subject of debate with cyclic loading and with internal and external humeral rotation to better simulate postoperative...
